NSW Government, Australia, Safety Alert from June 2008. While operating a RAM car (rubber-tyred mobile plant) in an underground coal mine, the driver heard a bang and noticed a distinctive smell and white smoke-like substance coming from the rear of the machine. A brown smoke-like substance followed.
Root cause analysis highlighted issues around rubber liner inserts in wheel assemblies and recommendation are made to prevent recurrence of similar incidents. Recommendation include:
- Confirm mine planning does not require vehicles to operate beyond their rated capacities (including for tyre tkph)
- Match fitted tyres to equipment capacities
- Control the information related to tyre capacities and confirm it is included in maintenance and training resources
