Escalated outcome from a mobile equipment fire, caused by:
- Exit blocked e.g. by fire or aftermarket siting of mobile equipment components
- Operators/workers unable to see through smoke
- Emergency exit blocked by equipment etc.
- Operator/worker trapped or unconscious after collision or rollover
- Inability to activate emergency exit due to unapproved or poorly communicated modification, defect or absence of required tooling
- Delayed emergency response to help with operator/worker response
- Inadequate systems to initiate emergency response (i.e. cannot call for help)
