This group of failure modes relates to errors made by mobile equipment operators and workers in proximity to them. The types of failure mode identified include:
- Not following required road rules;
- Making an error and driving on the wrong road for the class of vehicle being operated;
- Working when not fit (fatigued, affected by drugs, etc.);
- Not deploying seat belts when required;
- Travelling with limbs/body parts outside the confines of the vehicle;
- Changing behaviour as a result of alarms/devices deployed in the vehicle (fatigue alarms, proximity alerts, etc.), and
- Poor parking practice.
