This group of failure modes relates to errors made by mobile equipment operators and workers in proximity to them.  The types of failure mode identified include:

  • Not following required road rules;
  • Making an error and driving on the wrong road for the class of vehicle being operated;
  • Working when not fit (fatigued, affected by drugs, etc.);
  • Not deploying seat belts when required;
  • Travelling with limbs/body parts outside the confines of the vehicle;
  • Changing behaviour as a result of alarms/devices deployed in the vehicle (fatigue alarms, proximity alerts, etc.), and
  • Poor parking practice.

The business inputs recommended/identified as addressing these problems are listed.
