This group of failure modes relates to degrading of equipment due to the actions of operators – which will lead to impacting on the performance of mobile (and ancillary) equipment. The types of failure mode identified include:
- Failing to conduct pre-starts;
- Disabling safety devices;
- Confusion arising from multiple, conflicting, alarms;
- Operators ignoring alarms due to multiple nuisance alarms occurring.
