This group of failure modes arises from poorly considered or executed planning of works. It includes:
- Setting up works when prevailing conditions are unfavourable (high winds, poor vision, etc.)
- Requiring tasks to occur with conflicting travel routes/equipment mixes
- Not considering interaction issues when planning
- Lack of consideration of regulatory or third party constraints
The business inputs which address these types of failure modes are presented.
