- Inadequate breaks between shifts
- Shift patterns including travel time
- Inadequate consideration of social factors e.g. religious observations, community norms for alcohol and other drug use
- Failure to correctly identify potential for impairment arising from the use of mobile phones and/or other personal electronic devices
