An incident occurred in May 2019 where a manned excavator moved into the path of a semi-autonomous dozer. This provisional incident report highlights:

  • importance of barricades between vehicles of different types (not in place at time of incident
  • autonomous and semi-autonomous vehicles should have functional collision avoidance systems (not fitted)
  • requirements for monitoring of workers conducting semi-autonomous tasks (largely not in place – inspections were field based rather than functional based)
